Patient Rights & Responsibilities

AXON Medical Ambulatory Centre is committed to provide best healthcare solutions and quality care to our patients regardless of age, race, language, religion or social status. Patients can voice concerns and recommendations without concern for discrimination, retaliation or unreasonable interruption of care or treatment.

Our patients have the rights to:

Timely and Adequate Information

  • Be given adequate medical information such as medical condition, diagnosis and treatment plan.
  • Entitled to a Medical Report upon request and payment of a reasonable fee.
  • Entitled to be informed on the estimated charges for services provided.

Choices

  • Entitled to refuse a recommended treatment.
  • Entitled to request for a second opinion.

Other rights

  • Entitled to identify staff through staff uniform, badges, tag or staff identification chart.
  • Entitled to know the name and professional status of staff providing care or treatment.

Grievance

  • Entitled to submit any grievances or complaints via verbal or written form.

Confidentiality and Privacy

  • AMAC maintains strict confidentiality on information related to patient.
  • Information disclosed to the Medical Centre is only for purpose of treatment planning for patients and/or for Medical Centre’s administrative purposes.
  • The Medical Centre however may under certain circumstances compelled by law/relevant authority to disclose the information.
  • Patient’s written authorization is required in all other instances should there be a need to release the information.

AMAC’s patient also has the responsibilities to:

  • Patient/guardian shall provide accurate and complete information about his/her medical condition, medication and treatment history to the attending doctor to the best of their ability.
  • To provide patient’s personal details including contact number truthfully.
  • Patient shall adhere to medical advices and instructions should they choose to accept the treatment plan.
  • Patient shall keep to his appointment and inform the doctor/Medical Centre if he/she is unable to do so.
  • Patient shall adhere to rules and regulations, including to the policies of the Medical Centre.
  • Patient shall inform the Medical Centre of his/her ability to pay for the services to be rendered.
  • Patient shall pay their medical bill accordingly upon receiving the treatment/services.
  • While in the clinic or Medical Centre, patient and their companion/visitor shall respect and be considerate to others
